Agreement regarding Therapeutic Activities
Although this form can be used with clients in sex offender treatment, this form can be used with voluntary and involuntary clients in all forms of forensic treatment. This form may be used to create a contract between a treatment provider and client regarding such issues as treatment progress, treatment termination, treatment planning, attendance, excused absences, fees, collection policy, testimony, and the client's relationship with other clients in the program.
Behavior Contract
This form is used with clients in a sex offender treatment program. This form can be used to create a contract between a client and treatment provider in which the client agrees to control precursors of sexual misconduct, deviant sexual misconduct, and illegal sexual misconduct.
Informed Consent for Assessment
This form is used to gain informed consent by a mental health professional performing a forensic assessment. It addresses such issues as relationship to the referring party, purpose and goal of the assessment, nature of services, potential side-effects, limits of confidentiality, potential testimony, mandatory reporting, fees, collection policy, and cancelation policy.
Informed Consent for Forensic Treatment
This form can be used to gain informed consent from a client in any form of forensic treatment, including sex offender treatment, by informing the client about such issues as relationship with referring agency, how forensic treatment differs from traditional treatment, purpose and goal of treatment, side-effects, limits of confidentiality, potential testimony, mandatory reporting, use of an attorney, and terminating treatment.
Social History
The social history form is a twenty page, 439-item form completed by a client entering sex offender treatment. If the last section of the form, pertaining to sexual history, is removed, this form can be used with forensic and clinical clients who are not sex offenders. This form is comprised of eleven categories: (1) pregnancy, birth & infancy; (2) health; (3) current functioning; (4) self-regulation; (5) mental health; (6) childhood issues; (7) family of origin; (8) education; (9) vocation; (10) financial; (11) leisure & hobbies; (12) accommodations; (13) friends; (14) support systems; (15) religious & spiritual; (16) alcohol use; (17) drug use; (18) relationship history; (19) legal history and attitudes; (20) community supervision; and (21) sexual history.
Social and Electronic Media Policy
This form is used to teach clients about and obtain an agreement about your policies regarding electronic communication including texting, emails, Facebook and other forms of social media.
Treatment Plan Form
This treatment plan form can be used with voluntary and involuntary clients in treatment for sexual deviance or sexual offending. The treatment plan form is based upon research regarding dynamic risk and it addresses five areas: intimacy, social influences, sexual self-regulation, general self-regulation, and cooperation with supervision.

Policy and Procedure Manual for Community Supervision Sex Offender Program
This policy and procedure manual can be used by probation or parole departments to structure a community supervision sex offender program. Based upon the containment model and the good life model, this policy and procedure manual addresses the following issues: program philosophy, contractor requirements, structure of containment team, supervisee rights and welfare, referral and assessment process, treatment planning, offense specific therapy, psychoeducational therapy, polygraphs, and program evaluation.
